So, I am working on a 2013 kia forte with a 2.0L engine. It has a P2191 code ( Lean at higher load) So far I have checked for vacuum leaks, evap leaks, PCV system, fuel injectors, fuel pump, air filter, fuel leaks, F/A ratio sensors, O2 sensors, MAP. This car is not equipped with mass air flow sensor. When I first started working on this car, it had a leak from the valve cover gasket and the intake runner switch was missing and had a code for that. I went ahead and replaced the valve cover gasket and installed the intake runner switch. The code for the intake runner switch went away and it's no longer leaking. I noticed when I removed the valve cover, the cylinder head was very dirty with oil deposits. It was thick and just nasty. I wonder if that has anything to do with it.
The long term fuel trim is at 29%. When it's driven and hard on gas, it drops to 21% and sit there. When at idle and revved the long term fuel trim gets better but climbs back up as soon as you let go the gas pedal. I have a smoke machine so, I was able to smoke it for leaks. The car idles smooth and has plenty of power when driving, even when under high load.
Now, when I reset the fuel trim values, The LTFT drops to zero but the STFT would be very high at around 36% After a short test drive, the STFT drops down close to zero and the long term fuel trim is back again at 29%.
The intake manifold runner sensor that I installed on the vehicle was not tested at all. I just installed it cleared the code and the code never came back.

The symptoms would fit restricted fuel delivery, restriction on the fuel flow rate

Great! I will definitely be focusing of that. I have not checked the fuel line yet, that is from the pump to where the fuel rail starts but, as far as the fuel pump, fuel filter, injectors and the regulator they are all functioning properly.

In addition to fuel pressure/volume, take a fuel sample to test for excessive ethanol content. Was the tank filled recently?
