I'd try either using a stud extractor. or double nutting, the lower stud to see if the nut will unscrew. Or grabbing the steel "ring" right below the bushing with pliers might work if the nut ain't tight.

Just cut the old grommets off the bolt sleeves.
I like to use side cutters, but it really doesn't matter.
Then lube up the new grommet with white lithium/dielectric/silicone/whatever, and it'll slide right onto the sleeve.

No, just the rubber part. The bolt and the sleeve will stay intact and together. Then slide the new rubber part onto the bolt and sleeve.
