Great, just wanted to verify. :) It sure seems like you have a failing O2 sensor.

My only concern is for the B1S1 wiring. I never saw the B1S1 signal show more than 545mV, so it's possible that you're seeing some kind of high resistance in the sensor signal wire. I'd suggest doing a circuit integrity test on the B1S1 signal wire first, before replacing that sensor.

That's typical of data from Global OBD. Not to worry. :)

Even before it goes into closed loop? Or only after it goes into closed loop?

No worries. 545mV is the same as .545V. Just moving the decimal point around.

P2195 returned. I have the screen freeze. I checked fuel rail pressure. 2003 explorer 4.0 at 65 psi quickly dropped to 60psi and dropped to 35 in 2 hours.
I wasn't able to find a vacuum leak with carb cleaner.

Have you tried anything else out on this? If not, maybe I can help.

I think it's time to back up here a minute. For right now, forget about vacuum leaks, fuel pressure, etc. You have a code, or codes, for one particular sensor. Vacuum leaks and fuel pressure are most likely going to affect both banks.

The thing that's strange to me is P2195 and P2196 are both for the same sensor, but the codes are completely opposite of each other.

I am not familiar with the scan tool you are using, but do you know if it's capable of graphing? If so, can you put all 4 oxygen sensors on the screen. For right now there doesn't have to be any other data. Just do this. If you can record it, or even a picture of the screen, go out and floor the car and let it go through a 1-2 shift at WOT. The sensors should all go rich. If you can do this, post your picture and let's take a look at it.

Also, you said a muffler was installed with no tailpipe. Did the problem start only after that, or is that a coincidence? Is there a chance someone did some welding and used the oxygen sensor bung as the ground for the welder?

I cleared codes and a few weeks later only p2195 returned. I have the freeze frame data and I cleared it again. To see what returns. This weekend I was going to try fuel filter replacement, even though fiel pressure on rail at idle was 65 psi
No welding was done, used like jb high temp or extreme weld .
It seems like it bucks and jerks and takes 10 seconds to go from 60 to 70mph.
Yes the codes were all same bank but my understanderstanding not necessary the o2 sensor needs replacement even though it's an o2 code
My guesses are fuel filter, cat CONVERTER CLOGGED, FUEL INJECTOR.
I DONT REALLY UNDERSTAND LIVE DATA.
