Ford E450 2009 with 5.4L.... Flex fuel....
Ran across this problem today.... Hard start when cold, if you give it gas, stalls....
No codes but an alcohol content of 78%.... Despite always putting gas....
Did a KAM reset to relearn alcohol content.... Managed to take it down to 49%....
The real fix: Fuel pump was able to deliver 60 psi but not under load.... under load was more like 40psi...Unable to deliver the volume needed!
Fuel pump replaced and alcohol content relearned.... Now back to 0% and runing like a champ!

Keep in mind that the truck ran great after a couple of minutes after starting....

Yes, most newer Gm has them... But ford seems to calculate on fuel trims and injection time....
Once the pump was changed, and reset KAM, the fuel trims went way up! and the alcohol content went strait down in a matter of minutes...
The fix was the pump.... Customer was very happy to have his truck fixed! Always rewarding when they come to you and say: We know who to trust from now on!
