I have ruled out by testing. Torque converter, maf, dirty throttle body fuel pump, plugs, vtec and vcm solenoids and switches. The only testing discrepancy I can find is the following. When comparing TP voltage to APP %, from a chart in Mitchell, I'm dead on at 0% and 100% but at 25,50 and 75% it's off by 200mv. Could this be the issue? The whole range is only 4V so 200mv amounts to about 5%. Is that enough to confuse the Pcm?

My first thought is a tear in the intake boot between the MAF and throttle body. Hondas don't set trouble codes for this issue quickly like other makes.
I know you said you eliminated the torque converter from your list of suspects, but to be honest, the only other thing I have ever seen cause that kind of jerkiness of that generation Honda with that drivetrain has been converter shudder.
And even then, a fluid flush following manufacturer recommendations and a tune of shudder guard additive cured the symptoms.
Usually they are quite good at picking up misfires, and the throttle and accelerator circuits are closely monitored for issues.
