I've heard many different approaches to doing A/C retrofitting like this. One shop I worked for just sucked the R-12 out and put R-134 in.
Others will replace most of the system and flush the lines as well as possible. It all depends on who you talk to. :lol:
Personally, I'd suggest replacing the compressor and accumulator/receiver drier, as these hold the majority of the refrigerant oil. I've been told that R-12 oil becomes acidic when combined with R-134 oil, and can cause all kinds of problems if allowed to mix. Use pressurized A/C flush wherever possible, and suck a strong vacuum for at least 30 mins before filling it up.

I did this a year or so ago on my mom's 1993 Accord while on a short visit. The system was pretty much empty so I replaced the receiver dryer. I simply went to the auto parts store and bought the conversion "kit" they had on the shelf, rented a vacuum pump and gauge set, and followed the instructions in the kit. I think I had to buy an extra can of refrigerant to get enough for the system. The kit came with a can or two of refrigerant with oil already in it. I did not replace the compressor and I did not flush anything as the kit didn't require it. I was a little worried about mixing the oils but did not have time to do a full flush of the system. I did replace the schraeder valves before installing the new R134a fittings as I could tell that's where the system had been leaking. I am 2000 miles away from her but she messages me every now and then and says "My AC is still blowing cold and I don't have to use rags anymore for defrost in the winter. Thank you!"
If I had more time I would have done the full flush but it is still working after a little over a year. She knew I was a bit skeptical of how long it would last but it's done pretty well so far.
