What would cause a readiness monitor to not set? Any insight is greatly appreciated.

That it's going to vary depending on the year make and model and which monitor is not ready.
Temperature, fuel level, driving conditions and stored fault codes are a few reasons for monitors to not run.
"Ground cannot be checked with a 10mm socket"
